These are available to download on our website, so you can complete them before your appointment if you wish. Alternatively, you are welcome to complete them when you arrive at our clinic. We ask that you arrive a few minutes early to complete this paperwork if this is the case. You will need your Medicare number and information about any private health insurance funds.

It is important to complete the forms fully, as the information may affect our recommended treatment. We will need a complete list of all medications currently taken, including prescription and over-the-counter medicines and supplements. We also need to know about any pre-existing health conditions to ensure any procedures provided are as safe as possible.
